Friends and Family Line
NaphCare, our medical provider, offers a Friends & Family Line that provides family members and loved ones with a direct way to communicate with medical staff about a patient. The patient must complete a valid Release of Information form, and it must be on file. Whether you need to discuss a medical concern or provide details about a loved one's health history, NaphCare nurses are available to assist.
Purpose
The Friends and Family Line provides assistance for loved ones seeking or sharing meica information related to a NaphCare patient.
Release of Information Process
In compliancce with HIPAA laws, a patient-completed Release of Information form must be on file before information can be shared with friends and family.
Calls and requests to the Friends and Family Line will be answered by a dedicated team of nurses with access to up-to-date information
